Application

The following forms are required for our files upon enrollment:

• Student Emergency Card
• Application
• Emergency Treatment Release Form (notarized)
• Signed Financial Agreement
• Current health and immunization record

All forms must be complete and submitted with the registration fee and tuition payment.

Belleair Montessori Academy is committed to working with families and recognizes that there must be cooperation between the school and home for children to be successful. You have access to the school for observation and visitation of your child as long as it does not create a major disruption of the program. Drop-in visits by parents/guardians are welcome at any time. You may schedule an appointment with the director to discuss your child's development, accomplishments or behavior. Belleair Montessori Academy provides ongoing communication and opportunities for involvement in the school. Please refer to the monthly newsletter for upcoming events and special announcements. Your involvement is important for your child's success.
